COKC is excited to bring back our OFA Health Clinic at the Mid-Ohio Cluster!!!
Our health clinic will be located in the Bricker Building at the Ohio State Fairgrounds on Sunday, November 17, 2024 from 8:00 AM to 3:00 PM for Cardiac and Eye clinics, 10:00 AM to 3:00 PM for the RFGS Clinic.
Pre-registration and pre-payment can be made here on the webstore or sent to the address listed on the forms. Once payments and registration is received, you will be contacted by telephone or email to schedule an appointment time. Please check your emails for appointment confirmation! Our clinics fill up fast & walk-ins will ONLY be worked in when possible. Pre-registration is strongly recommended!

The Cardiology Clinic will be conducted by Dr. Robert Hamlin, DVM and Dr. Steve Roof.
Pre-Paid Pre-Registration (non-refundable):
Auscultations are $60
Electrocardiograms (ECG) are $70
Echocardiograms (includes Auscultation) are $280
Day of Clinic Fees:
Auscultations are $70
Electrocardiograms (ECG) are $80
Echocardiograms (includes Auscultation) are $300
Dogs must be 12 months or older to receive a Cardiac OFA number.
Advanced exam forms for the ECG and Echo will be available for completion at the clinic. The form for the Auscultation exam can be downloaded at the OFA website. You will need to know the following for each dog: AKC registration number and Microchip number.
Auscultations take roughly 5 minutes, ECG’s take roughly 10 minutes, and Echos can take 15-20 minutes.

OFA Basic Cardiac Application for Auscultations
The Eye Clinic will be conducted by Dr. Bill Greentree, DVM
Pre-Paid Pre-Registration (non-refundable):
$50 per dog
Australian Shepherd, Basenji, Pembroke Welsh Corgi, and Dalmatian require an examination before AND after dilation: $55 per dog
Day of Clinic Fees:
$55 per dog

OFA Forms will be available from Dr. Greentree at the clinic. You need to bring the following for each dog to be examined: AKC Registration Number & Microchip Number.
Eye exams take roughly 5 minutes. Please plan to show up at least 15-20 minutes before your scheduled appointment to have your dog’s eyes dilated with the drops.
*Planning your appointment time: A dog’s pupils start constricting again within 2-3 hours after drops. Dog’s with later ring times should be examined first thing in the morning. Pupil dilation does not affect the vision in dogs like it does in humans because they do not accommodate (use the muscle in the iris to fine focus their vision). It can make dogs a little light sensitive until the pupil can constrict some which is more of a concern at outdoor shows in sunny days.

Respiratory Function Grading Scheme (RFGS) Clinic by Dr. Amanda Bolyard, DVM

***The screening program is currently open to Pugs, Bulldogs, and French Bulldogs. Other brachycephalic breeds can participate, however they are ineligible at this time for certification. ***
***Please Note: All owners are encouraged to use OFA online to pre-register for RFGS exams as follows: Login to your OFA online account, or if a new user, register for an OFA online account. Select “Create new application,” and enter your dog’s information. Scroll down and select Online Exams, select Respiratory Function Grading Scheme and COKC’s health clinic information. Complete the brief pre-clinic health survey for your dog. Complete the $15 payment to cover the OFA registration fees. This OFA fee does not include the examination fee that is paid separately to COKC. Once your online form is completed, you will receive an email confirming registration and a PDF attachment of the RFGS exam form. A paper copy of the RFGS exam form will be available for walk-in appointments.***
Planning your appointment time: There are multiple steps to this exam including a pre-examination, an exercise test that lasts 3 minutes, and a post-examination. Please arrive 5-10 minutes early for your scheduled exam to check-in with clinic volunteers and to make sure your paperwork is in order. If you are late to your scheduled appointment, we may not be able to fit you in for a later exam. If you are planning to do a walk-in appointment, please arrive NO LATER than 2:00 PM to check for appointment availability. Exam fees are NON-REFUNDABLE.
